Specialists in Plastic Surgery, PA will use the personally identifiable information submitted by you solely for the purpose for which you have provided it, whether it be to subscribe to a service, request information, etc. We may also use the information you provide to us to contact you for reasons related to your original communication to us. Additionally, we may periodically send you promotional e-mails about existing or prospective products and services offered by us and our advertisers, as well as to obtain information to better understand our users. Postal Address Collection and Use
If you supply us with your postal address on-line, we will use it to send you the information for which you provided us your address. If you supply us with your telephone number on-line, we will use it to contact you with information regarding your account, orders you have placed, or with information regarding your inquiry. We are not responsible for files (including but not limited to photos, content, images, et. al.) provided by our customers and used on their websites or in their printed materials.
Medical Record Destruction Public Notice
In keeping with current state and federal regulations governing the retention and destructions of patient health records, notice is hereby given of our intent to destroy, by shredding, the records of those individuals who last received treatment prior to July 31, 2010 and who are not currently active patients.
Destruction of medical records is scheduled to begin October 1, 2020. You may contact our medical records department at 919-785-0505 if you wish to pick up your medical records before destruction is set to begin.
