Who Should Get Ear Surgery?

Candidates for ear surgery should be healthy and have fully developed ears, which usually occurs around five to six years of age.

Otoplasty can treat:

The goal of ear surgery is to enhance the natural proportions of the face and restore a sense of balance. The reshaped ears should be symmetrical and should emerge from the head at a natural angle, without a pinned-back appearance. Both ears are typically treated during the same surgery.

What to Expect During Your Ear Surgery

The techniques used during ear surgery will vary depending on the type of correction being performed. General anesthetic or intravenous sedation will be administered to ensure your comfort during the procedure. During ear angle adjustment, incisions are made behind the ear, hidden in the natural ear folds. Cartilage and tissue are removed or reshaped so the ear position can be adjusted.

Once the improved position has been achieved, the incisions are closed with fine sutures for minimal scarring. In some cases, incisions will be necessary on the front of the ear, as well. When at all possible, incisions will be hidden within the natural folds of the ear.

After Your Surgery

After the procedure, gauze or bandages are applied to protect the ears from infection. A head band or compression dressing is applied for the first few weeks to maintain the new position. You may experience some discomfort in the first few days, which can be managed with over-the-counter medication. Some swelling and bruising is likely, which resolves over the course of a few weeks.

In most cases, normal activities can be resumed in 7-10 days, though strenuous activity should be avoided for at least a few weeks. The ears should be protected from accidental impact for the first few months. Incisions lines will fade in approximately 12-18 months after ear surgery.

Understanding the Risks of Ear Surgery

Nose Reshaping

Nose surgery, also known as rhinoplasty, is a surgical procedure to enhance the appearance of your nose. For some patients, it also improves the function of the nose, especially if the person suffers from a deviated septum or other structural abnormality. The nose is the focal point of the face, and changes to it can have a powerful effect on the overall balance of your face—even with just minor surgical adjustments.

Who Should Get Rhinoplasty?

The best candidates for rhinoplasty are in good overall health and possess realistic expectations about what nose surgery will be like and what it can do for them. Patients have to be old enough to have completed facial growth, which generally means that they have reached at least 14 years of age. However, your surgeon will need to consult with each patient directly in order to assess whether or not he or she is a candidate.

Rhinoplasty can treat:

What to Expect During Your Nose Surgery

When you have nose surgery, it will require general and/or local anesthesia in an outpatient surgical setting. We begin by making incisions within the nostrils or across the base of the nose. We will then separate the skin from the underlying soft cartilage and bone. Specialized tools will help your doctor reshape the underlying structure of your nose.

After nose reshaping, all incisions are secured with sutures. A cotton dressing or soft nasal splints will be placed inside of your nasal passage, as well as an external splint on the outside to support and protect your nose.

After Your Surgery

You will need to stay home and keep your head elevated after surgery in the days following surgery in order to minimize swelling. The bruising and swelling will be its worst during this time, but after about a week or so you will start to notice it slowly subside. Within a couple of weeks, the bruising should resolve completely and swelling should resolve gradually over the next two months.

You will need to wait 10-14 days before you resume your normal activities, and, when you do, you will need to protect your nose from impact and sun exposure. Strenous activity needs to be avoided for several weeks.

Fat and skin are removed from within incisions, and a laser may be used to tighten and firm the skin beneath the eye. The incision is closed using very fine sutures for minimal scarring.

After Your Surgery

Following surgery, an ointment will be applied to the eyes, which may blur vision initially. Some swelling and bruising is likely, which will further puff the eyelids. Discomfort should be minimal. The first few days should be spent with the head elevated and cold compresses applied to the eyes. There may be a tight feeling around the eyes at first. This discomfort can be treated with over-the-counter medication.

Stitches are removed after the first week, and bruising should go down in that time, as well. Some swelling may persist for one to three weeks. Normal activities may be resumed in a week to ten days, though caution should be taken not to strain the eyes or do the heavy lifting. Concealing makeup may be applied at this time if desired.

All Surgery Carries Some Uncertainty and Risk

Complications are rare and usually minor when a forehead lift is performed by a qualified plastic surgeon. Yet, the possibility of complications must be considered.

In rare cases, the nerves that control eyebrow movement may be injured on one or both sides, resulting in a loss of ability to raise the eyebrows or wrinkle the forehead. Additional surgery may be required to correct the problem.

Formation of a broad scar is also a rare complication. This may be treated surgically by removing the wide scar tissue so a new, thinner scar may result. Also, in some patients, hair loss may occur along the scar edges.

Loss of sensation along or just beyond the incision line is common, especially with the classic forehead lift procedure. It is usually temporary, but may be permanent in some patients.

Infection and bleeding are very rare, but are possibilities.

You can reduce your risk of complications by closely following your surgeon’s instructions both before and after surgery.

Planning Your Surgery

For a better understanding of how a forehead lift might change your appearance, look into a mirror and place the palms of your hands at the outer edges of your eyes, above your eyebrows. Gently draw the skin up to raise the brow and the forehead area. That is approximately what a forehead lift would do for you.

If you decide to consult a plastic surgeon, he or she will first evaluate your face, including the skin and underlying bone.

During your consultation, the surgeon will discuss your goals for the surgery and ask you about certain medical conditions that could cause problems during or after the procedure, such as uncontrolled high blood pressure, blood-clotting problems, or the tendency to develop large scars. Be sure to tell the surgeon if you have had previous facial surgery, if you smoke, or if you take any drugs or medications — including aspirin or other drugs that affect clotting.

Preparing For Your Surgery

Your surgeon will give you specific instructions to prepare for the procedure, including guidelines on eating and drinking, smoking, and taking and avoiding certain vitamins and medications. Carefully following these instructions will help your surgery and your recovery proceed more smoothly.

If your hair is very short, you may wish to let it grow out before surgery, so that it’s long enough to hide the scars while they heal.

Whether your forehead lift is done in an outpatient facility or in the hospital, you should arrange for someone to drive you home after your surgery, and to help you out for a day or two.

Where Your Surgery Will Be Performed

A forehead lift is usually done in a surgeon’s office-based facility or an outpatient surgery center. However, it is occasionally done in the hospital.

Anesthesia Used for The Procedure

All forehead lifts are performed under general anesthesia, in which case you’ll sleep through the entire operation.

 

The Surgery

Your surgeon will help you decide which surgical approach will best achieve your cosmetic goals: the classic or open method. Make sure you understand the technique that your surgeon recommends and why he or she feels it is best for you.

The classic forehead lift

Before the operation begins, your hair will be tied with rubber bands on either side of the incision line. Your head will not be shaved, but hair that is growing directly in front of the incision line may need to be trimmed.

For most patients, a coronal incision will be used. It follows a headphone-like pattern, starting at about ear level and running across the top of the forehead and down the other side of the head. The incision is usually made well behind the hairline so that the scar won’t be visible.

If your hairline is high or receding, the incision may be placed just at the hairline, to avoid adding even more height to the forehead. In patients who are bald or losing hair, a mid-scalp incision that follows the natural pattern of the skull bones is sometimes recommended. By wearing your hair down on your forehead, most such scars become relatively inconspicuous. Special planning is sometimes necessary for concealing the scar in male patients, whose hairstyles often don’t lend themselves as well to incision coverage.

If you are bald or have thinning hair, your surgeon may recommend a mid-scalp incision so the resulting scar follows the natural junction of two bones in your skull and is less conspicuous.

Working through the incision, the skin of the forehead is carefully lifted so that the underlying tissue can be removed and the muscles of the forehead can be altered or released. The eyebrows may also be elevated and excess skin at the incision point will be trimmed away to help create a smoother, more youthful appearance.

The incision is then closed with stitches or clips. Your face and hair will be washed to prevent irritation and the rubber bands will be removed from your hair. Although some plastic surgeons do not use any dressings, your doctor may choose to cover the incision with gauze padding and wrap the head in an elastic bandage.

After Your Surgery

Classic forehead lift patients may experience some numbness and temporary discomfort around the incision, which can be controlled with prescription medication. Patients who are prone to headaches may be treated with an additional longer-acting local anesthesia during surgery as a preventive measure.

You may be told to keep your head elevated for two to three days following surgery to keep the swelling down. Swelling may also affect the cheeks and eyes– however, this should begin to disappear in a week or so.

As the nerves heal, numbness on the top of your scalp may be replaced by itching. These sensations may take as long as six months to fully disappear. If bandages were used, they will be removed a day or two after surgery. Most stitches or clips will be removed within two weeks, sometimes in two stages.

Some of your hair around the incision may fall out and may temporarily be a bit thinner. Normal growth will usually resume within a few weeks or months. Permanent hair loss is rare.

Incision site pain is usually minimal, but can be controlled with medication, if necessary.

The stitches or staples used to close the incisions are usually removed within a week and the temporary fixation screws within two weeks.

Getting Back to Normal

Although you should be up and about in a day or two, plan on taking it easy for at least the first week after surgery. You should be able to shower and shampoo your hair within two days, or as soon as the bandage is removed.

Most patients are back to work or school in a week to 10 days. Prolonged exposure to heat or sun should be limited for several months.

Most of the visible signs of surgery should fade completely within about three weeks. Minor swelling and bruising can be concealed with special camouflage makeup. You may feel a bit tired and let down at first, but your energy level will increase as you begin to look and feel better.

Your New Look

Most patients are pleased with the results of a forehead lift, no matter which surgical method was used. Often, patients don’t realize how much their sagging forehead contributed to the signs of aging until they see how much younger and more rested they appear after the lift.

Although a forehead lift does not stop the clock, it can minimize the appearance of aging for years. As time passes, you may want to repeat the procedure.

Your surgeon will manipulate or remove some of the muscle to improve contour.

After Your Surgery

Recovery time varies. Bandages will be applied to the treatment area following surgery, and a compression garment may be worn for the first few days to decrease swelling and improve circulation. There will be some discomfort immediately following the procedure, but this can be managed using prescription medication.

What to Expect During Your Facial Fat Grafting

Prior to your procedure, the donor locations are marked for fat removal. Donor fat may be liposuctioned from trouble areas in the hips, thighs, abdomen, arms, or buttocks. These areas are cleansed and small incisions are made in hidden locations. A liposuction cannula or low pressure syringe is then inserted into the incisions. The fat is carefully withdrawn, followed by incision closure.

Before injection, the fat is purified and prepared. The recipient sites are marked and cleansed. Fat is injected into these areas at varying depths to form an entire grid of injected fat. Massage may be used at the end of treatment to ensure even distribution.

After Fat Grafting

After fat grafting, standard dressings, tape, and compression garments are applied to the treatment area. Although some swelling and bruising is likley after the procedure, over-the-counter pain medication should be sufficient to manage discomfort. Swelling and bruising should resolve after two to three weeks. Some fat is usually absorbed naturally by the body, so the injection sites may be slightly over-filled at first to compensate for this effect.
